LIFE CHOICE TERMS AND POLICIES
Qualification of a Whole Sale Account:
·        Life Choice requires a business license, GST# and / or registration number.
·        A minimum of $2,500.00 in annual sales.
Head Office Hours, Statutory Holidays and Weekends:
Life Choice head office and warehouse are open Monday to Friday 9:00am to 5:00pm MST. Life Choice head office and warehouse are closed for all statutory holidays and on weekends.
Placing Orders:
Orders can be placed by phone, fax, e-mail or online. Please state your store name, phone number and/or customer number. Please order by product name or code and quantity. If calls are received after hours or the lines are busy, please leave a voicemail with your store name and phone number, and we will return your call as soon as possible.
Order Minimums:
Free shipping order must be $500.00 net (or more) before tax and after all discounts are applied.
Minimum order is $250.00 net, before tax and after all discounts are applied.
Orders under $200.00 are subject to a shipping and/or handling fee with some exceptions approved by H/O.
Shipping Orders:
All orders are shipped within 2-3 business days.
All Life Choice orders will be shipped with either FedEx Ground Canadian Parcels or upon special request with Canada Post.
Overnight shipping is available upon request at full shipping price only. Overnight shipping must be requested and paid by 11:00 am MST – Mon. to Fri. The average Canadian over night parcel cost can reach up to $85.00.
 Shipping Addresses:
Business addresses are preferred. For residential businesses and delivery, a business license must be faxed/emailed to the Life Choice head office before the first purchase order is placed. Residential shipping addresses are subject to an extra $15.00 shipping surcharge without any exception. This fee will be applied even on orders that have reached the minimum for Free Shipping or even on Free Shipping Days. No exceptions.
Free Shipping, No Minimum Days:
Free Ship Days are offered on certain dates each month. If the regular date falls on a weekend or a holiday, Free Ship Day will be applied the Friday before or a different date will be assigned for the Free Shipping/ No Minimum Day. Life Choice Head Office will send out, newsletters, email and fax notification if this happens.
No minimum of any kind is required on any order on this day.
The breakdown of Free Ship/ No Minimum Days in Provinces are as follows:
Manitoba (MB), Ontario (ON), Quebec (QC), Maritimes (PEI, NB, NS, NFLB) – 20th of each month. Saskatchewan (SK), Alberta (AB), British Columbia (BC) – 25th of each month.
Qualifications for Participating in Free Shipping Day: Clarification and to reiterate, a minimum of $2,500.00 in annual sales are needed to qualify for a wholesale account.
Payment Options:

·        Prepaid payment by Visa or MasterCard
30 day net terms: All requests for terms must be approved by Life Choice Head Office Management. Cheque or bank transfer should be received within 30 days. A credit card may be kept on file in case of late payment(s). New customers must establish a payment history before requesting 30 days net terms. Consistently late payments may result in revocation of terms.
COD: Payable by money order or cheque upon receipt of order.
Online wholesale orders: Credit card payment only   (www.lifechoicewholesale.net )
NSF cheques: Accounts with NFS (not sufficient funds) will be charged a $25 bank fee.
Services: Life Choice will not service outstanding accounts.
Distributors:
Life Choice Products can be purchased with Purity Life (519) 853-3511or  www.pirutylife.ca and Horizon Distribution (604) 524-9964 or www.horizondistributions.com. Customers please be advised; orders via distributors have to follow the appropriate company’s terms and policies.
Discounts, specials, case stack deals:
Standing discounts: Requests for standing discounts must be approved by Head Office: a baseline dollar amount of sales is usually required to be eligible for a standing discount. Please note, accounts with standing discounts are ineligible for other discounts (case stack deals, CHFA, 3+1 new sku promo, etc.).
Monthly promotion: Â Please contact Life Choice head office for details on any monthly promotions.
Case stack deals: Case stack discounts are automatically applied to all purchase orders: 12 units/sku – 5% off, 24 units/sku – 10% off, 72 units/sku – 15% off, 144/sku – 20% off. Please contact Life Choice head office for any higher quantity discounts. No mix and match of any sku’s are allowed.
New SKU promo: When ordering a new product (sku) the first time, Life Choice offers Buy 3 Get 1 for Free. This promotion only applies for the first three purchased units.
Opening orders: Â Life Choice offers 10% off on any opening orders. No minimum required.
CHFA: Life Choice offers special line drive discounts with each CHFA Trade Show. For the discount details please contact the Life Choice Head office or look for the CHFA flyer that is promoted prior to the show.
Discounts, offers or promotions cannot be mixed or combined.
Returns:
Store returns from consumers: Please contact Head Office with the customer’s name, phone number and reason for return. Life Choice head office will issue a credit to your account, which will be applied to your next purchase order. Please contact Head Office for instructions on what to do with the returned product(s).
Store Returns (for exchange, short date and/or damage): If the product received is damaged or short dated, Life Choice will offer either an exchange or a credit towards your account. Please keep in mind that short dated products sold at a discount are final sale. For returns that are not customer returns or for which Life Choice is not at fault, a 15% restocking fee will be applied to the credit. The returned product(s) must be unopened and at least 12 months from expiry.
Discontinued items cannot under any circumstances be accepted for credit or product exchange.
Adjustments and Errors:
Please report any billing or shipping errors within 5 days of receiving the product. The proper adjustments will be made immediately.

Life Choice Ltd. reserves the right to change the wholesale prices and/or company terms and policies at any time. Life Choice Ltd. reserves the right to terminate any and all relationship with any account that deems unfit for a whole sale account.
